The Regence Foundation recently announced it awarded an $11,000 grant to the Free Clinic of Southwest Washington to establish a Transitional Diabetic Care Project. The project aims to help stabilize patients suffering from uncontrolled diabetes and help them transition into a medical home for long-term disease management.

The clinic, which is the largest free clinic in the state, provides basic health services to low-income populations on a walk-in basis.
